Community Support

Our Integrated Community Support programs provide coordinated, person-centered care that supports both behavioral and physical health. Through whole-person care, we promote resilience, recovery, independence, and overall wellness.

Meaningful relationships are central to this approach. Our Community Support Specialists, Nurse Care Managers, wellness specialists, and Occupational Therapists work alongside participants to identify barriers, build on personal strengths, and develop practical strategies for achieving their goals. By partnering across programs, we help address the health disparities that contribute to reduced life expectancy for people living with mental illness.

Healthcare Home services are fully integrated into this model and include annual health assessments, personalized health education and coaching, fitness support, care coordination between providers, and support following hospitalizations.

Participants know they have trusted professionals walking beside them every step of the way, empowering them to live with dignity, confidence, and greater independence in the community.

Our Services

  • Connections to essential community resources and services, including Medicaid, food assistance, psychiatric care, physical health care, medication coordination, and housing support.
  • Exploring housing options that provide the right balance of support, safety, and independence by partnering with both community-based housing providers and Independence Center Residential Programs.
  • Assisting with strengthening daily living skills such as maintaining a home, scheduling appointments, and managing everyday responsibilities that contribute to long-term wellness and success.
  • Providing crisis intervention, emergency assistance, and support for members and their families during times of need.

ITCD

Integrated Treatment for Co-Occurring Disorders (ITCD) is an evidence-based program designed to support individuals living with both a serious mental illness and a substance use disorder. By integrating mental health and substance use treatment into one coordinated approach, participants receive consistent, comprehensive care focused on recovery, wellness, and long-term stability.

Services are provided by a multidisciplinary team working together in one setting and include psychiatric care, nursing support, substance use treatment, community support services, and individual and group therapy. This integrated model helps participants build healthier coping skills, strengthen recovery supports, and achieve greater independence and quality of life.

CSC-FEP

Independence Center is the 4th provider in the State of Missouri to begin offering Coordinated Specialty Care for First Episode Psychosis, a unique multi-disciplinary model designed to support young people who have recently had their first episode of psychosis. Young people engaged in CSC programs are more likely to continue treatment, less likely to have side effects from medication, and are less likely to be readmitted to the hospital within the first year.

At Independence Center, 50% of young people in the CSC program are employed or enrolled in school. Since the program began in 2025, Independence Center has served 36 people in the CSC program.
