Options for filing a complaint are:

c. Request the form through the mail.

Complainants must file their signed, dated complaint within 60 days of the alleged incident. The complaint must include:
a. Complainant’s name, address and phone number
b. Detailed description of how, when, where the discrimination happened and what individuals or programs were involved. As much detailed specific information as possible should be included for the purpose of the investigation.
c. The names of other persons who should be contacted for clarity and details regarding the allegations of discrimination.
d. The completed complaint form or written complaint should be submitted to the address above or given to the Supervisor/Manager of the program.

Investigations
a. Independence Center investigates complaints received no later than 60 days after the alleged incident. IC will process complaints that include enough relevant information to warrant an investigation. IC will make best efforts to complete the investigation of the incident within 60 days of receiving it. If IC needs more information, the original complainant will be contacted and requested to provide additional information within 10 days.
b. IC reserves the right to administratively close the case if:
i. The complainant is not responsive to attempted contacts regarding the alleged discrimination.
ii. The complainant no longer wishes to pursue the complaint.
c. When an investigation is complete, IC will mail the complainant a summary of the findings and results with a corrective action plan, if one is warranted based on the findings. If the complainant is not satisfied with these results, IC encourages them to pursue the complaint with Missouri Department of Transportation or the Federal Transit Administration.
d. Complainants also have the option to file directly with these government bodies, if they choose to supersede the Independence Center investigation process.
